Informed Consent for Ear Wax Removal (Cerumen Management)

I acknowledge that I have been provided with information regarding the proposed procedure of ear wax removal (cerumen management), which may be performed using manual techniques including micro-suction and/or curettage.

I understand the purpose, nature, and expected benefits of the procedure. I have had the opportunity to ask questions and have received satisfactory answers.

I have been informed of the potential risks and complications associated with this procedure, which may include, but are not limited to:

  • Discomfort or irritation of the ear canal

  • Minor bleeding

  • Infection

  • Temporary dizziness or vertigo

  • Tinnitus (ringing in the ears)

  • Perforation of the eardrum (tympanic membrane)

  • Temporary or, in rare cases, permanent hearing changes

I understand that while all reasonable care and skill will be exercised, no guarantee has been made regarding the outcome of the procedure.

I confirm that I have disclosed relevant medical history, including any ear conditions, previous surgeries, or symptoms that may affect treatment.

I understand that the procedure will be performed by a qualified healthcare professional operating within their scope of practice.

I acknowledge that I have the right to withdraw or withhold consent at any time prior to or during the procedure without affecting my future care.

I have read and understood the above information and consent to proceed with the treatment.


If No Ear Wax is Found

Please note a 50% discount applies if there is no ear wax found in the appointment. The audiologist will give you advice regarding your symptoms and will have two options:

  1. Quick Hearing Check on the day - they will assess you hearing on the day and the total appointment fee = $110 or $120 (weekends)

  2. Consultation Only - If you choose not to check your hearing, the consultation fee = $55 or $60 (weekends)

The initial visit discount is because you’ve already had the consultation, therefore you can add a quick hearing check for only $55

A referral to other health professionals will be arranged, if appropriate.

Cancellation Policy​

Clear Ears Perth is a locally owned, family run business. If you need to cancel or reschedule your appointment, please do so 3 hours before the appointment start time. We understand that circumstances change and we are more than happy to be accomodating if you need to cancel or reschedule your appointment.

We often have people on our wait list hoping to secure a booking for that day who would be very grateful for a spot opening up.

If you do not contact the clinic at all to let us know you are unable to attend, a '$50 no show attendance fee' will be invoiced to you. If you pay this fee, it will be deducted from your next appointment (total fee minus $50).
